In part 1, Dr Imogen Staveley talks with Ben about how doctors use simulation training and checklists to avoid mistakes, maximise communication, and make sure that systems improve. She discusses how medicine learned directly from aviation as another industry with complex problems, little time to spare, and high risk situations.

How do you move from a culture valuing individual heroic effort and experience to one which spreads learning around teams and embraces mistakes? Medicine is still on its journey, but there’s a lot for here us to learn from.
